Terms and Conditions

Auto-Replenishment Terms and Conditions

  1. Your subscriptions will continue until you cancel it.
  2. You can modify and cancel your next order by updating it online.
  3. A recurring charge in the amount of the product selected for auto-replenishment will be automatically charged to your credit card for each shipment on the schedule you requested when you selected the product.
  4. There is no minimum term of the subscription and no minimum purchase obligation.

All prices reflect discounts and taxes (if applicable). Additional charges may apply. No other discounts may be used when purchasing services or products. If not deemed medically appropriate, Clevens Face and Body Specialists has the right to refuse treatment and refund payment. No other refunds or exchanges will be given. Services are non-refundable. Laser Skin Resurfacing packages do not include additional touch-ups. Appointments must be scheduled and initial services must be rendered within six months of purchase. Once scheduled, a cancellation fee of $75 will be applied if appointments are not cancelled and rescheduled with at least 48 hours notice. The cancellation fee will be deducted from the amount of your credit for pre-paid procedures and services listed above.
